Rehabilitation Services: St. Anthony’s Healthcare and Rehab has partnered with Triton Healthcare to provide an array of comprehensive integrated rehabilitation programs to its residents.  Program goals include:  Restoration of patient functioning, Improvement in quality of life, and the return to the least restrictive setting appropriate for the patient’s need.

Nutritional Services: All meals are planned by Registered Dietitian to meet nutritional requirements.

Hospice Care: Comfort Care and Pain Management care near the end of life

Respite Care: Short term care, allowing the primary home caregiver personal time

Care Plan Conference: Our interdisciplinary team will work with the resident and family to develop a person centered plan of care that meets their specific needs and goals.
