Southeast Community Health Systems (SCHS) is a Federally Qualified Health Center that provides primary and preventative care in four rural-based communities in Southeast Louisiana. As a private, non-profit 501(c)3 organization, SCHS provides medical, dental, podiatry, behavioral health, which include counseling, medication management, and substance abuse treatment. SCHS offers health screenings, acute care for minor illness and injury, comprehensive exams, and immunizations. SCHS accepts Medicaid, Medicare, and most private insurances. SCHS offers a sliding fee scale based on income. SCHS has certified Medicaid enrollment specialists that can assist in completing the LA Medicaid Application.
