HighPoint Hospice and HighPoint Palliative Care are teams of dedicated professionals that provide social, emotional, spiritual, and physical care & support to persons with life-limiting conditions (confirmed by their Primary Care Provider) to maintain optimal comfort & quality of life through utilizing our interdisciplinary team: Medical Director, Registered Nurses, Nurse Practitioner, CNA's, Social Workers, Chaplains, Bereavement Specialists, and trained Volunteers.  Our plan for the person's care is driven by the person (if they are able to make their choices known) or by the caregivers. Hospice & Palliative Care is provided where the person lives. Hospice & Palliative Care are services, not places. The Medicare Benefit covers all Hospice services 100%. There is NO COST to a person for Hospice Care. There is NO TIME LIMIT for Hospice Care unless the person no longer meets the Hospice criteria. The Hospice Team does not stay in your home 24 hours, but makes intermittent visits each week and is available 24 hours for symptom management or troubleshooting issues.  Please call 615-328-6690 for additional information on Hospice and Palliative Care programs.

Who pays for Hospice Care?

Hospice is covered by Medicare, Medicaid, and most private insurance. Hospice is available to individuals with end-stage illness for which there is no cure. But hospice care is not limited to the last few days of a patient’s life. In fact, hospice is designed to provide support throughout the final stages of illness.
