TARCOG is expanding some programs at the Area Agency on Aging (AAA), covering the counties in the northeast corner of the state. Additional slots were made available in the Medicaid Waiver program, which provides in-home care to frail seniors and other disabled people who would otherwise be eligible for nursing home care. Case Managers are busy out in the community doing assessments of people who might qualify. If you know of someone you would like to refer for such services, the phone number to call is 256.830.0818.
There is also a new program that will see TARCOG working in partnership with AQAF and four area hospitals: Athens-Limestone Hospital, Crestwood Medical Center, Highlands Medical Center, and Huntsville Hospital. The program will work with Medicare beneficiaries who are being discharged from these hospitals to give them the support services they need to best recover. There are three primary goals of the program: to maximize patient health, to reduce hospital readmissions, and to save Medicare dollars.
